VO2max Explained: Why Your Aerobic Fitness Predicts How Long You'll Live
VO2max is the single best measure of cardiorespiratory fitness — and one of the strongest predictors of longevity ever measured. Here's what it means, why it matters so much, and the training that reliably raises it at any age.
Ask a longevity researcher which single number they would want to know about your future health, and a surprising number will not say your cholesterol or your blood pressure. They will say your VO₂max — the maximum amount of oxygen your body can use during hard exercise. What VO₂max Actually Measures
VO₂max is the maximum rate at which your body can take in, transport, and use oxygen during intense effort, usually expressed in millilitres of oxygen per kilogram of body weight per minute (ml/kg/min). It is a whole-system score: it reflects how efficiently your lungs load oxygen, how much blood your heart can pump, and how well your muscles extract and burn that oxygen. A landmark study of more than 120,000 people undergoing treadmill testing found that higher fitness was associated with progressively lower mortality — with no observed ceiling of benefit. The least-fit group had dramatically higher mortality than the fittest, and the researchers noted that the risk of low fitness was comparable to or greater than traditional risk factors like smoking, diabetes, and hypertension.

This is not a single-study fluke. A meta-analysis of healthy adults found that each 1-MET increase in fitness (roughly 3.5 ml/kg/min of VO₂max) was associated with about a 13% lower risk of all-cause mortality and a 15% lower risk of cardiovascular events. Because of evidence like this, a major cardiology body has argued fitness should be treated as a clinical vital sign.

What Counts as a “Good” VO₂max?
Absolute values depend heavily on age and sex — VO₂max naturally declines with age and is on average higher in men than women — so a single universal target is misleading. A sedentary older adult might sit around 25 ml/kg/min, a recreationally fit adult in the high 30s to 40s, and an elite endurance athlete well above 60. As with heart rate variability, the number that matters most is your own trajectory: fitness typically drops about 10% per decade after your 30s, but consistent training can slow, halt, or reverse that decline — effectively buying back years of fitness age.
The Training That Reliably Raises VO₂max

Important Caveats
A wrist-based VO₂max estimate is not a laboratory measurement — treat the absolute value with humility and follow the trend rather than obsessing over a two-point change. High-intensity intervals are demanding; if you are new to training, have a heart condition, or have been sedentary, build an aerobic base first and talk to a clinician before adding all-out efforts.
